Insurance
Our practice participates in most insurance plans, including Medicare. As a participating provider with Medicare, we accept their assignment for claims processing. Please note that we do not participate with Medicaid. If you are insured with a plan that we do not participate with, payment in full is expected at the time of service. If you are unsure as to our participation with your plan, please contact our office so we can assist you. If we participate with your plan, coverage and benefits are determined by that plan. It is your responsibility to know your insurance benefits. Please contact your insurance company with any questions you may have regarding your coverage and how your claims will be paid.
Copayments are a contractual obligation you have with your insurance company and must be paid at the time of service. If your insurance plan requires that you obtain a referral prior to being seen, you will need to make sure we receive one, prior to your appointment. If you come to your appointment without a referral you will not be seen by the doctor unless you sign a waiver and pay for your visit.
If we have your current insurance information, we will submit your claims and assist you in any way we can in order to help you get your claims paid. Your insurance company may need you to supply certain information directly to them. You will need to comply with their request. If you do not supply the requested information in a timely manner, you will be responsible for the balance on your account.
If your insurance coverage changes, please notify our office as soon as possible so that we can make the appropriate changes to your account.
Payment Options
We accept VISA, Mastercard, Discover and AMEX for payment.
If your account is over 90 days past due, your account may be sent to our collection agency for review.
Patient Forms
All patients must complete the patient information forms before seeing the doctor. A copy of both your valid driver's license and a current insurance card will be needed by this office for your medical record. If you fail to provide us with the correct insurance information in a timely manner, you will be responsible for the charges for your visit.
Appointments/Cancellations
Please help us serve you better by keeping your regularly scheduled appointment. Our most important concern is your vision. You are being seen here for a retinal problem. Many of these conditions can progress without obvious symptoms or change in vision. Periodic monitoring of your condition is essential to minimize your risk for vision loss so it is imperative that you see Dr. ____. as instructed. We understand that scheduling changes do occur, but we ask that you kindly give us at least 24 hours notice if you need to reschedule.
